Diabetes UK MK Voluntary Group next meeting: Thursday, 18th October 2018, 7pm at Centrecom Community Centre, 602 North Row, Secklow Gate West, Central Milton Keynes, MK9 3BJ


 

DESMOND (Diabetes Education and Self Management for Ongoing and Newly Diagnosed) is a course for people with Type 2 diabetes.

The newly diagnosed module is for those within 12 months of diagnosis and the Foundation module is for those with established diabetes

The referral form should be on S1, will self populate from the patient record and should be sent in by secure email. 

Please note that ‚Äčthere is very little wait to attend a course so referrring Health Care Professionals should ensure they have received the results of tests shown as mandatory on the referral form, before sending it in.  The patient and the educators need this information in order to participate fully and be supported during the course. 

We have course venues in Willen, Wolverton and Bletchley and courses available on weekdays, in the evenings and on Saturdays.  Patients are sent an invitation letter offering choices, within 48 hours of receipt of referral.
